What is Case Management?

Case Management is the collaborative process of assessment, planning, facilitation, care coordination and advocacy for options and services to meet an individual’s health needs through communication and available resources to promote quality cost effective outcomes. [1]
 
Case Management is a team effort consisting of the client, carer, Case Manager, formal and informal support networks and service providers.
 
Case Managers provide a single point of contact for clients, helping the client access and navigate the service system, optimising use of available resources on either an ongoing, short term or episodic basis. They work with and for the individual, enhancing their independence, confidence and control.
  • AHC Case Managers seek to empower clients by recognising they are capable, have strengths and resources that can help them take control of their lives  
     
  • AHC Case Managers treat clients respectfully, assisting them to identify their needs, build on their strengths whilst supporting them to meet their goals through individualised care planning
 
Why use a Case Manager?
AHC have professional, experienced staff that can provide support in coordinating a wide range of health and community services that enables clients to continue living at home.
 
  • A Case Manager can reduce the stress and anxieties of coordinating and managing a wide range of services and supports into the home
     
  • A Case Manager works collaboratively with clients and families, setting realistic care goals, facilitating and identifying appropriate care and support
Following a full consultation with the client, services are tailored to meet the client’s needs. Services provided may include direct services from AHC and/or a range of complimentary services from contract service providers.
  
What can AHC Case Management offer?
  • Assistance to remain living at home with a level of confidence and support clients require  
  • Development of care plans that identify and support clients requirements
  • Advocacy
  • Assistance with health promotion and education
  • After-hours telephone support service
  • Referral to alternative support services
  • Residential Aged Care assistance (transition into or out of residential care, arrangements for shopping/banking/companionship services)
  • Monthly billing statements

  • High quality home or personal care
  • Allied health professional services e.g. physiotherapy and occupational therapy
  • Coordinating respite care, mental health and community services
  • Recreational and social support
  • Educational and employment services
 
AHC Case Management Services can assist clients while they wait for a Government funded care package and/or a quick response to help them coordinate services in their home.
 
Flexible service options available depending on individual needs:

Cost:
Case Management is billed at an hourly rate and invoiced monthly. A Schedule of Fees for services are available on request.
